Care Connect brings a person's history, relationships, and own voice into care and cognitive support settings, so care teams and family can stay connected to who someone has always been.

The Problem
Cognitive and memory-related conditions can make it harder for someone to tell their own story in the moment. Care teams often meet a person only through a file: medications, routines, a short intake form. The person's history, humor, and relationships rarely make it into that file.
Families carry the weight of filling in that gap, over and over, with every new caregiver and every shift change. And when a loved one cannot easily recall a name or a memory themselves, it can feel like a piece of them is slipping away, even when the person is still very much present.
Care Connect exists to keep that history close at hand, for the person, their family, and the people caring for them.
What It Does
With consent from family or an authorized representative, a person's stories, relationships, and preferences are organized so care teams can meet the whole person, not only their current needs.
A consented, evidence-backed likeness, built from a person's own voice, images, and memories, that can support gentle, familiar conversation and moments of reminiscence, scored for reliability by the Forever Spoken Trusted Multi-layered Fidelity Index. We call it the Fidelity Index from here on.
The Conversation Lounge gives family a way to stay in short, low-pressure contact, without needing to schedule a call around a care routine.
Personal and family information is handled with consent-based sharing and role-based access, built for the sensitivity of a care setting from the start.
How It Works

Family or an authorized representative shares history and preferences, with consent recorded at every step, since the person receiving care may not always be able to consent for themselves.

Care staff receive a reviewed, curated view of the person's history, appropriate to their role, so it supports care without overwhelming a shift.

Staff and family can use the record to guide gentle, familiar conversation, and every interaction is handled with the same care as the memories it draws on.

Where a person cannot consent for themselves, a family member or legally authorized representative consents and documents that authority before any record is built.
Each detail traces back to a real source: a family member's account, a recording, a document. Nothing enters without a record of where it came from.
Staff see only what is appropriate to their role. Family controls who else may be invited in.
A family or authorized representative can update permissions, restrict access, or take a record offline at any time.
Care Connect describes privacy-aware handling and governance, not a regulatory or clinical status.
